Sharing my experience in adding a new smart fob key to replace one I lost.
There are a few other threads on this but none I could find specifically for a smart fob key.
In this case I have a 2016 Limited (push button start) but would imagine it works for all Gen 5 smart key fobs.
The stealership quoted me at over $450 for them to do it. I did it for a total of $210 with an OEM smart key fob.
Bought the smart key fob on Keylessride.com for $180. Key is same as stock.
You will need 3 other items to make this work.
1) OBDII USB cable to connect a laptop to your trucks OBDII port. You need this specific version

2) Software that should cost $15: Techstream OBD2 Toyota Lexus TIS Software V12.20.024. You can find it all over such as eBay.
3) Laptop with Windows.
You need to get the exact or newer versions of the software and cable I mention for it to work on newer laptops (windows 10, 32bit or 64bit). Older versions will only work on older versions of Windows.
Install the Techstream software and the driver for the Mini VCI OBDII cable (should come with the cable) on your laptop.
Open the TIS software and select Mini VCI under the Setup menu so the software knows to use the cable. (Screenshot below)

It worked flawlessly and I now have 2 working fobs again!
You can also easily configure for many other customizations such as turn off the seatbelt chime, roll all the windows and moon roof down by holding unlock, etc.
